2016.31 updated with new clickable Table of Contents

Updated manual (same version 2016.31). New navigation buttons added.

Kevin wrote: "Main TOC is now clickable to the chapters.  Chapter headings return you to the chapter TOC.  Clicking the xLights logo will return you to the main TOC.  Clicking the logo on the TOC returns you to the cover."
